MD Thesis Abstract

Cold storage, although simple and efficient, has reached its limit with respect to the duration, preservation and the quality of organs that it achieves. In addition it does not allow for adequate viability assessment whereby a transplant team can safely predict whether an organ is capable of sustaining life in a recipient. There is good evidence that preservation times could be extended by the provision of continuous cellular substrate without a concomitant increase in preservation/reperfusion injury. Simulation of in-vivo conditions by ex-vivo perfusion could also mean that marginal organs will be salvaged for transplantation thereby expanding the ever diminishing donor pool.

This body of work aimed to compare the current gold standard of liver preservation, namely cold flush and storage in University of Wisconsin solution, with normothermic extracorporeal perfusion in a large animal study. A liver perfusion circuit was utilised not only as a method of achieving normothermic sanguineous preservation but also as a method of analysing different aspects of liver function during the reperfusion phase, following both warm and cold preservation

Preliminary experiments centred on optimising the configuration of the circuit components so that liver perfusion can be achieved to maximum effectiveness. This included a study exploring the need for bile salt replenishment during lengthy perfusions. Subsequently a series of experiments were performed that compared a 24 hour period of normothermic preservation with an identical period of cold preservation using the circuit as a surrogate for the transplant recipient, analysing components of synthetic, metabolic, excretory and haemodynamic function in the isolated liver. This series of experiments was performed in livers with and without pre-existing warm ischaemic injury reflecting both the non heart beating donor as well as the cadaveric brain dead donor respectively.

Teaching and Management

As an anatomy lecturer at St Marys Hospital I taught undergraduate medical and dental students and was responsible for their ongoing assessment. This involved both marking written material and anatomy viva-voce examinations.

Since that time I have continued to teach clinical students attached to surgical firms in Cambridge, Oxford, London and Birmingham. Teaching was both ward and lecture based with a practical and theoretical element. I was also the final year surgical tutor at St Marys and the John Radcliffe Hospital Oxford. I have helped to set up and examined final MB examinations in Oxford, as well as delivered several formal lectures as part of a pancreas module to 3rd year clinical students in Birmingham. I was also responsible for setting up the clinicals for a second part FRCS exam at St Marys Hospital.

I have also been involved with teaching surgical junior doctors, aiming specifically to prepare them for their MRCS examinations. I was responsible for setting up a weekly journal club at the North Staffordshire hospital as preparation for the academic component of the intercollegiate exam.

I am a faculty member for the CCrISP (care of the critically ill surgical patient) course on behalf of the Royal College of Surgeons and have taught and examined on this course. I have also taught on a MRCS course at the North Staffordshire Hospital. I was an invited speaker at the most recent British Transplant Society (BTS) Summerschool giving an overview of Liver Transplantation to a predominantly postgraduate audience.

For the last 2 years I have been a member of the Training committee of the British Transplant society and as such have been involved with drawing up a template for higher surgical training in liver/kidney transplantation in line with ongoing reforms including Modernising Medical Careers (MMC). This also allows me to act as a voice on behalf of my surgical colleagues within this specialised area with regard to training issues.

I was also the Secretary of the Carrel club (National Transplant Surgical Trainees Society) from 2003 – 2005 and am partially responsible for its current resurrection from complete dormancy to currently over 50 members. I believe it is essential for trainees to have a voice in the current climate of change and potential “dumbing-down” of surgical training within the UK and was the instigator for an annual national meeting which exists independently of the BTS. I organised the most recent one held this year in Birmingham (Nov 2005) and attracted over 30 delegates and a nationally renowned faculty. I have also been invited to speak in the forthcoming Annual BTS meeting in Edinburgh representing the Carrel club, and the surgical chapter meeting of the BTS in Manchester Nov 2006.

I was responsible for organising and running the on call rota on the Birmingham liver unit over the last year. This involves co-ordinating retrieval teams, theatre and ward doctors as well as visiting fellows on the unit, as well as organising leave for juniors. I have regularly co-ordinated and managed outpatient priorities, waiting lists and theatre Sessions.
